Register with us as a New Patient

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

You will need to complete a registration form, and also attend an appointment with a nurse to have a health check which will provide useful information. It will take between 5 and 10 working days before you can make an appointment with one of our nurses. Only then will you will be registered with our practice. You will be unable to have a normal appointment, have medication or medical certificates issued prior to you having your nurse health check. 

If you do not attend your nurse health check appointment you will not be registered with us.  

Note: We are not accepting patients who already have a registered GP in the Hamilton area

Once registered your medical records will be transferred from your previous doctor.

All patients between 16 and 75 who have not seen a doctor in the past 3 years are offered a health check. This check-up is usually carried out by the Practice Sister.

All private medicals are undertaken by the doctor.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
